The Components of Neck Aging

An attractive neck is defined by a sharp, clean jawline and a well-defined 90-degree angle beneath the chin. Over time, three major anatomical changes can disrupt this profile:

  1. Muscle Separation (Platysmal Bands): The platysma is a thin sheet of muscle covering the front of the neck. As we age, its left and right sides can separate and sag, creating prominent vertical “cords” or bands on the throat.
  2. Submental Fat Accumulation: Genetic predisposition or aging can cause a stubborn pocket of fat to gather directly under the chin (often called a double chin), obscuring the jawbone.
  3. Skin Laxity: A natural loss of collagen leads to thin, inelastic skin that hangs loosely, often referred to as a “turkey neck.”

What Happens During Neck Lift Surgery?

Step 1: Incisions & Access

Depending on your unique anatomy, the surgeon will make a small, well-hidden incision under the chin (about 3 cm long) and discreet incisions that wrap closely around the earlobes and hide in the crease behind the ears.

Step 2: Submental Liposuction & Fat Sculpting

Through the small chin incision, excess fat deposits lying above and below the neck muscles are carefully sculpted or suctioned out to redefine the jawline.

Step 3: The Platysmaplasty (Muscle Corset)

The surgeon accesses the separated edges of the platysma muscle. These edges are brought together in the center and meticulously stitched together from the chin down to the base of the throat. This works exactly like an internal corset, flattening out vertical bands and creating a sharp structural angle.

Step 4: Skin Redraping and Trimming

The overlying skin is gently smoothed and swept upwards and backwards behind the ears. Any redundant, loose skin is precisely trimmed away.

Step 5: Closing

Incisions are closed using ultra-fine sutures placed within natural skin creases to ensure that the eventual scars are virtually imperceptible.
