  • The Target Area: The buccal fat pad is a distinct pocket of fat located deep within the lower cheek, beneath the cheekbones.
  • The Problem: Large buccal fat pads are typically genetic and do not go away with diet or exercise. They can cause a round, soft, or permanently “baby-faced” appearance, even in individuals who are at a healthy weight.
  • The Goal: The procedure does not remove all cheek fat; it simply reduces the volume of this specific pocket to hollow out the lower face slightly, making the cheekbones and jawline appear more prominent.

Who is an Ideal Candidate?

Because facial fat naturally diminishes with age, patient selection is crucial for a long-lasting, elegant result.

  • Good Candidates: Individuals with naturally round, full faces (“chipmunk cheeks”) who desire a more angular, contoured look, and who possess good skin elasticity.

Step-by-Step: What Happens During Surgery?

Step 1: Complete Numbing

The area is thoroughly numbed using local anesthesia. If combined with other surgeries, sedation or general anesthesia may be used.

Step 2: Hidden Incisions

The surgeon makes a small incision (about 1 to 2 cm) on the inside of the cheek, near the upper molars. There are absolutely no external scars on the face.

Step 3: Precision Removal

The surgeon applies gentle external pressure to the cheek, causing the buccal fat pad to naturally tease out through the small internal opening. The precise amount of fat is carefully sculpted and removed.

Step 4: Dissolvable Closure

The tiny internal cuts are closed using dissolvable stitches. These do not need to be removed and will fade away on their own within a week.
